    Metabolic energy requirements for space flight

    The international space community, including the USSR, Japan, Germany, the European Space Agency, and the US, is preparing for extended stays in space. Much of the research planned for space will be tended by humans, thus, maintaining adequate nutritional status during long stays in space has lately become an issue of much interest. Historically, it appears that minimum nutritional requirements are being met during stays in space. Thus far, crewmembers have been able to consume food adequate for maintaining nominal performance in microgravity. The physiological data obtained from ground-based and flight research that may enable us to understand the biochemical alterations that effect energy utilization and performance. Focus is on energy utilization during the Apollo lunar missions, Skylab's extended space lab missions, and Space Shuttle flights. Available data includes those recorded during intra- and extravehicular activities as well as during microgravity simulation (bed rest). Data on metabolism during flight and during bed rest are discussed, with a follow-up on human gastrointestinal function

    This chapter focuses on the history, development, and perceived value of The Open University UK’s BA/BSc (Hons) Open degree (hereafter referred to as “OUUK Open degree”) over the past half-century in the context of changing external pressures and addressing debates around the coherence and acceptance of such a personalized program of study. It touches on the changing views of “openness” over time, from the origins of The Open University’s “open entry” policy, through to ideas around flexibility of study, open education, and personalized learning. The chapter concludes with recommendations for other higher education institutions wishing to introduce a multidisciplinary open degree into their portfolio of curriculum

    The Influences of Caregiver-Child Interactions and Temperament on Cortisol Concentrations of Toddlers in Full-Day Childcare

    The purpose of this study was to explore the influences of caregiver-child interaction and temperament on cortisol concentrations of toddlers in full-day childcare. Sensitive and responsive caregiving is critical to the quality of children\u27s early experiences. Caregivers who are attuned to unique needs of young children can support and guide child growth and development. In the context of relationships the child grows, develops, and learns to biologically respond to events in his or her world that he or she may perceive as threatening, with the production of cortisol. Studies show that regulation of cortisol release later in life may be shaped by social experiences during early development. This exploratory study investigated the influences of caregiver-child interaction and temperament as measured by the Early Childhood Behavior Questionnaire (ECBQ) on cortisol concentrations in toddlers in full-day childcare. Seventy-three (31female, 42 male) toddlers, in 11 full-day childcare classrooms in communities in southern Appalachia participated. Classrooms were evaluated using the toddler Classroom Assessment Scoring System (CLASS) and an adapted version of the Engagement Check II. Morning and afternoon cortisol concentration was measured in enzymeimmunoassays of saliva. Findings indicated that as years of teacher experience increased, cortisol concentrations decreased. Likewise, as the percentage of time toddlers were engaged in developmentally appropriate activities increased, cortisol concentration decreased. Mean cortisol concentrations decreased from mid-morning to mid-afternoon. Findings of individual cortisol concentrations were mixed. Temperament data showed a relationship between management of arousal or impulse control (surgency) and cortisol concentration. No statistically significant correlation was found between cortisol concentration and the dimensions of the toddler CLASS. Regression analysis of the 3 dimensions of temperament (negative affect, effortful control, surgency) showed surgency to be related to cortisol concentration. The study adds to the body of research on very young children in full-day childcare and elevated cortisol concentrations by including children in communities in southern Appalachia as well as measuring teacher-child interaction in childcare using the newly released toddler CLASS. Future research is needed to delineate the developmental outcomes and long-term impact of excess stress in this population

    Legacy of Environmental Research During the Space Shuttle Program

    The Space Shuttle Program provided many opportunities to study the role of spaceflight on human life for over the last 30 years and represents the longest and largest U.S. human spaceflight program. Risks to crewmembers were included in the research areas of nutrition, microbiology, toxicology, radiation, and sleep quality. To better understand the Shuttle environment, Crew Health Care System was developed. As part of this system, the Environmental Health Subsystem was developed to monitor the atmosphere for gaseous contaminants and microbial contamination levels and to monitor water quality and radiation. This program expended a great deal of effort in studying and mitigating risks related to contaminations due to food, water, air, surfaces, crewmembers, and payloads including those with animals. As the Shuttle had limited stowage space and food selection, the development of nutritional requirements for crewmembers was imperative. As the Shuttle was a reusable vehicle, microbial contamination was of great concern. The development of monitoring instruments that could withstand the space environment took several years and many variations to come up with a suitable instrument. Research with space radiation provided an improved understanding of the various sources of ionizing radiation and the development of monitoring instrumentation for space weather and the human exposure within the orbiter's cabin. Space toxicology matured to include the management of offgassing products that could pollute the crewmembers air quality. The Shuttle Program implemented a 5-level toxicity rating system and developed new monitoring instrumentation to detect toxic compounds. The environment of space caused circadian desynchrony, sleep deficiency, and fatigue leading to much research and major emphasis on countermeasures. Outcomes of the research in these areas were countermeasures, operational protocols, and hardware.     Nutritional Requirements for Space Station Freedom Crews

    The purpose of this report was to set preliminary nutritional requirements for crewmembers flying from 90 to 180 day missions on Space Station Freedom. Specific recommendations included providing crewmembers with in flight feedback on nutritional intake, weight and strength, and incorporating issues of energy intake, body weight, body composition, strength, and protein intake in the flight medicine program. Exercise must be considered an integral part of any plan to maintain nutritional status, especially those modes that stress the skeleton and maintain body weight. Nutrient intake, amount of exercise, and drugs ingested must be recorded daily; high priority should be given to development of fully automated record systems that minimize astronauts' effort. A system of nutritional supplements should be developed to provide a method for reducing intake deficits that become apparent. Finally, post flight monitoring should include bone density, muscle mass and function, and iron status at three and six months after landing

    Thromboprophylaxis in atrial fibrillation and association with cognitive decline: systematic review

    Objective:Atrial Fibrillation (AF) is associated with dementia. If AF-related cognitive decline is driven by cerebral embolic events, thromboprophylaxis may impact on this. This systematic review assessed the association between cognitive impairment and AF thromboprophylaxis. Methods:Two independent reviewers searched CINAHL, EMBASE, MEDLINE, PsycINFO, Web of Science Core Collection, and Cochrane Library from inception until 12th November 2014. Eligible studies compared AF thromboprophylaxis to control with an outcome measure of cognition or dementia. Where data allowed, meta-analyses describing between-group differences in cognitive test scores or rates of incident dementia were performed. Results:Nineteen studies were eligible. For two prospective studies (one RCT) comparing anticoagulation against antiplatelet therapy, change in Mini-Mental State Examination score from baseline to last follow-up (maximal duration:5.9 years) suggested a difference favouring anticoagulation (mean difference:0.90, 95% CI:0.29 to 1.51), in keeping with a trend seen in the single RCT (mean difference MMSE:0.80. 95% CI:-0.07 to 1.67). Pooled odds ratios suggested no association with incident dementia, comparing anticoagulant to antiplatelet therapy (two studies, OR:1.23, 95% CI:0.80 to 1.91) or no treatment (three studies, OR:0.89, 95%CI:0.47 to 1.69). Conclusions:Our analyses show no definitive evidence of cognitive benefit or harm from anticoagulation. We demonstrated a potential benefit of anticoagulation in comparison to antiplatelet over time. Larger-scale studies with longer follow-up are needed to determine the true cognitive impact of AF thromboprophylaxis
